How does ERP work?

ERP (Exposure and Response Prevention) is a proactive, evidence-based treatment that works by identifying, disrupting, and dismantling the cycle of chronic anxiety, OCD, and phobias.

Traditional talk therapy often tries to “reason” with your fears. ERP goes directly to the source. It targets the amygdala—the brain’s emotional alarm center—to teach it that the things you fear are not actually dangerous. By changing how you react to a thought, you eventually change how you feel about it.

  • The Exposure: Under the guidance of your therapist, you gradually and safely face the thoughts, images, or situations that trigger your anxiety (like the fear of germs or the “what if” of getting sick).

  • The Response Prevention: This is the most vital step. You learn to lean into the discomfort without performing your usual “safety behaviors”—the compulsions, checking, or avoiding that provide temporary relief but keep the fear alive.

  • Habituation: By staying present with the discomfort rather than running from it, your nervous system naturally begins to calm down. Your brain learns through experience that it can handle the uncertainty, leading to a permanent reduction in the power those fears hold over you.

    ERP is a collaborative, “roll-up-your-sleeves” type of therapy. We don’t just talk about your fears; we actively practice the skills you need to disarm them. Here is how our work together typically unfolds:

    1. Building Your “Fear Hierarchy”

    We start by mapping out your triggers. We’ll create a list of situations, thoughts, or objects that cause you distress and rank them on a scale from 1 to 10. This ensures we never start with your “final boss” fear; we begin with smaller, manageable challenges to build your confidence.

    2. Strategic Exposures

    In session, we will leaning into a specific trigger from your list.

    • In-Vivo Exposure: Facing a real-life situation (e.g., touching a “contaminated” doorknob or eating a “scary” food).

    • Imaginal Exposure: Writing or listening to a story about a feared outcome to desensitize your brain to the “what if.”

    • Interoceptive Exposure: Purposefully mimicking physical symptoms of anxiety (like dizziness or a racing heart) to prove they aren’t dangerous.

    3. Response Prevention (The Core Work)

    This is where the magic happens. While you are feeling the “spike” of anxiety from the exposure, I will coach you through not performing your usual ritual. Instead of washing your hands, checking your pulse, or seeking reassurance, we stay with the discomfort until it begins to fade on its own.

     4. Between-Session “Homework”

    Since OCD and phobias don’t stay in the therapy room, neither does the treatment. You’ll leave each session with a clear, agreed-upon plan to practice specific exposures in your daily life. This “real-world” practice is what creates permanent changes in your brain.

    A Note on Safety: You are always in control. We will never do an exposure that you haven’t agreed to, and we will never move faster than your nervous system can handle. My role is to be your “coach”—pushing you just enough to grow, while ensuring you feel supported every step of the way.
